Suitable for: all healthcare professionals working with diabetic patients in primary care.

Wednesday 17th April 2024 – 12.30 – 13.30 – Freestyle Libreview

Session 1 -  Introduction to Time in Range and glucose metrics.

Wednesday 24th April 2024 – 12.30 – 13.30 – Freestyle Libreview

Session 2 - Interpretation of AGP and Libreview reports through a simple 5 step process.

Wednesday  8th May 2024  - 12.30 – 13.30 -  Freestyle Libreview 

Session 3 – Case Studies – Consolidation

Session 1 – Introduction to Time in Range and CGM glucose targets – (1 hour)

 By the end of this session you will be able to:

• Describe the Triangle of Diabetes Care
• Explain how CGM differs from capillary blood glucose testing
• Understand what is meant by Time in range and how this relates to HbAlc
• State the targets and glucose metrics outlined by the International Consensus guidelines on Time in Range.
• Understand the anatomy of the Ambulatory glucose profile (AGP) report and other Libreview reports.

  
Session 2 –Interpretation of AGP & Libreview reports, through a simple 5 step process.  (1 hour)

 By the end of this session, you will be able to:

• Access individual patient glucose reports through the LibreView platform.

• Utilize a simple 5 step process to effectively identify and understand current glucose trends.
• Conduct an effective patient centered discussion – which supports you and your patient uncover what is contributing to areas of concern.
• Work with your patient to set clear goals and agree specific actions and therapeutic changes.

Session 3 -  Case studies and Consolidation (1 hour)

 By the end of this session you will:

• Have had the opportunity to discuss individual case studies.
• Share best practice with other HCP colleagues.
• Gain a deeper insight and practice in utilizing the 5 step approach to interpretation of AGP reports.

This session will be presented virtually in Teams, hosted by Abbott.  The Learning and Development Centre will send meeting information in the week prior to the course.  Please keep this information private and confidential.
