Shaping Future Care; Delivering the 10 Year Plan Through Digitally Enabled Prevention, Closer to Home Conference - 9th October 2025

We have been given the challenge by our Secretary of State to deliver healthcare that is Fit for The Future through three shifts in care:
- Analogue to digital
- Treatment to prevention
- Hospital to community
A key approach to addressing these is to reframe the ask as opportunities to strategically commission differently within and across neighbourhoods: to give your population the ability to manage their long term conditions that will, in turn, reduce the burden on primary and secondary care and, in turn, help to save money within your system. In building the infrastructure: technology, capability and capacity, the system begins to turn the curve on many challenges, for example UEC (Urgent Emergency Care).
At this conference we will be showcasing examples of innovation and creativity in designing and delivering patient care close out of hospital and close to home. You will hear about PCNs that have developed new ways of delivering care to their patients and embracing the opportunities for utilising digital technologies and data about our population taking the learning and application from the region’s long covid services and the Virtual Hub. This has evolved into an exemplar of virtual, holistic, personalised and neighbourhood care underpinned by providing access to digital tools.
Through an interactive day, you will have the opportunity to take part in discussions about how you can use the evidence to make the changes needed in your local system and meet with some suppliers of technical solutions to inspire and inform you.
Most of all, this is a safe space to encourage debate and promote networking with your colleagues from a range of systems and other stakeholders, including leaders from the regional NHS England team.
We want to give you the headspace and guidance to enable you to be creative in your thinking and explore how this model of care can work for your population.
We will do this by:
- Giving you real world examples of delivering innovative digital models of care and the opportunity to talk to the people who did it and learn from them.
- Providing you with opportunities to discuss with other system and healthcare leaders how you can make this work for your local population.
- Introducing you to the suppliers who can provide the technology you need.
- Providing you with the opportunity to listen to inspiring speakers – people with passion about the health system and a desire to do things differently
And, most importantly:
- Give you a moment of space along with others to stimulate your creativity to enable you to make the changes needed in your local population.
We’re here to identify opportunities for change and solve problems. It’s easier to solve them together.

actively participate and network, and form new connections to drive forward
their local and regional agendas.
There is plenty of free parking available at the venue. Refreshments and a buffet lunch will be provided during the day.
